Practice Policy:

Dr. Waneka  has written a letter to patients outlining the Practice Policy as well as describing the payment plans available. Please take a moment to read the policy statement, choose which plan is best for you and submit it to the office when you have decided.

We are currently contracted to accept only these insurance providers:

Blue Cross
Blue Shield
Aetna

Patients without coverage from these providers should settle their accounts at the time of their visit.  However, as a special service, we will be happy to submit the bill to your insurance company. Any reimbursements will then be made to you directly from your provider. Please call with any specific questions you may have. If you wish, you may fax or email your insurance information to us for verification of benefits.
